For your convenience, Piga Primary Care Associates have provided the following forms to save you time on your first appointment, and so that the doctor can see your child more quickly. If your child is already a patient, please let us know if any of your information has changed.
The following forms are editable PDFs. Each form can be downloaded, completed and either emailed back to our office or printed. If printed, please bring the completed form when you come in to our office for your appointment.
Medical Authorization for Minors
For parents to authorize another adult (more than18 years old) to bring a child in for an appointment and authorize treatment (for example: grandparents, nanny, babysitter, adult sibling, aunt, etc).
Release of Medical Records - Incoming
For patients transferring medical records to our practice.
Release of Medical Records - Outgoing
For patients transferring medical records out.
Patient Registration Packet
Our office uses Phreesia for all check in/ registration forms. Patients will receive a text and email to complete this online before their appointment. Please print and complete the packet above if you are unable to register via text or email.
